AF Amistad High School Family Handbook 2017-2018 | 24 If your child requires medication during school hours, please contact the School Nurse to request a Medication Administration Form (or MAF). Only the School Nurse has the training and resources to store and administer medication. However, medication cannot be administered to your child until your child’s medical provider has completed the form. This is a requirement of the CT Department of Education and pertains to all medicine, including Tylenol, and other over- the-counter medicines. The School Nurse will keep a detailed log of all medicines that are administered. In cases in which a scholar is ill and should not be in school – either because he/she presents a health risk to our members of our school community, or he/she is unable to be in class – the School Nurse will request a parent/guardian or emergency contact to pick up a scholar. In these instances, it is the responsibility of the family to arrange pick up as soon as possible. Sick scholars cannot be sent home unescorted via taxi or public transportation or on foot due out of concern for their safety. All scholars must have a pass from a staff member to visit the Nurse’s Office. This helps retain academic focus, and to ensure the safety of scholars (by making sure at least one staff member knows the scholar is feeling ill in case they don’t make it to the Nurse’s Office). Communication AFAHS requires parents to be partners in the education of their children. The important task of educating a child calls for the school, the student, and the parent(s)/guardian(s) to all work together to ensure success. Means of Communication AFAHS has a number of ways that it communicates with its families: AFAHS Website: The AFAHS website at www.afamistadhs.org is a valuable source of information for scholars and families. It features information about our policies and procedures, school and athletic calendars, staff contact information, and links to useful forms and documents such as permission slips. Infinite Campus Parent Portal Every student and parent at AFAHS is assigned an access code to log onto Infinite Campus Parent Portal, software which enables teachers to post grades, assignments, and announcements on line. Parent Portal also includes information on student attendance, debts to school, bus stop. It is the school’s expectation that students and families check Infinite Campus regularly to keep abreast of grades and announcements.
